Appointments & Consultations:

Cancellations:
If you are unable to keep an appointment, we ask that you kindly provide us with at least 48 hours notice. This courtesy on your part will make it possible to give your appointment to another patient.

Emergencies:
Our office provides on-call service for emergencies, 24 hours-a-day, 7 days-a-week. Call 941.365.2737 and Dr. Slazinski will be contacted immediately.

Health Insurance Plans:
Our office participates with most health insurance plans which cover certain procedures. Covered procedures will vary with each insurance company -- our office staff will assist you in determining eligibility. Patients are responsible for deductibles, co-payments and remaining balances. Patients also have the option of paying office fees in advance and then receiving their insurance reimbursement check directly from the insurance company.

Financing:
Most medical insurance plans will not usually cover cosmetic procedures. Our office is able to assist you with a financing program for elective medical procedures. Patients may be pre-approved for financing with a low-interest monthly payment plan. Our office manager would be pleased to provide you with further details. In addition, we also accept the following: cash, personal checks, Visa and Mastercard, as well as debit cards.

Gift Certificates:
Gift Certificates must be presented at time of arrival and are non-refundable.
